please help,Taking my Chihuahua for a vacation from India to USA

Hi,
I am planning to take my 10 months old Chihuahua with me from Mumbai (India) to JFK / EWR (USA) from May 16 to Nov.16
We will fly by Lufthansa since they allow small dogs in cabin.
What things I need to do in India as well as in US, while going and returning?
What documents and preparations I need ?
Please help.

SDM - Within 7 days of export from India, dogs and cats will need a health certificate from their vet as well as an Animal Health Certificate from the Animal Quarantine and Certification Service as well as all documentation required for their destination country. Before you leave, ask as to whether a DGFT license will be required to return as you will not be staying away for 2 years. As for the US, your pet will need proof of rabies vaccination at least 30 days prior to entry and a screwworm test between one and five days of entry.
