Pet Health Certificate Turkish TARIM to US

Hi--I'm bringing a pet to the US via a flight volunteer. Has anyone gotten a Pet Health Certificate from TARIM? Do they ask for your passport information, or what is the process like? Can I just provide name, phone number without a passport? Thank you!
