Kelly - an Annex I form is used when your pet is flying unaccompanied (commercially). There is another Annex form used if you or your representative is flying with or within 5 days of your pet. Whether your pet is flying with or within 5 days of you or your designated representative or it is flying unaccompanied, the USDA must endorse the form. The pet passport is a collection of documents required for your pet to enter a given country. They are taped on the crate or you carry them with you if your pet is approved to fly in the cabin.

So a vet needs to fill out the Annex I form and the USDA needs to endorse it. But the form needs to be filled out 10 days or less from the time of arrival in England. (the country I will be traveling to) When and to who do you provide the "pet passport" to??
